[MonoDevelop] MD 5.1 Makefile integration

Thanks, Andres,

you're right this addin and some others are disabled by default in
Monodevelop 5.1. To enable it I simply went to Tools/Addin managed and
clicked on it.  

I was aware that the feature was removed from monodevelop.  Thanks God, it's
not!
