[MonoDevelop] Reporting 4.1.13 bugs

Hi,

1. I just added the 4.1.13 version, thanks for pointing out that we didn't
have one yet

2. open a terminal and run `kill -QUIT <pid>` where <pid> is the
MonoDevelop process id and then submit a bug report with the Ide.log file
(it'll be wherever MonoDevelop saves its log files in Linux, I forget where
that is off-hand - probably ~/.config/MonoDevelop or something).

3. I'll leave that for someone else to answer because I don't know
