Description of Problem:

When installing monotools-1.1.4868.vsix on Windows Vista x86_64 with VS 2010
where monotools is already installed, I get the following error:

  "This extension is already installed to all applicable products."

You need to un-install the plugin first before you can upgrade.  This needs to
be upgradeable.
