Precompiled test case with source and configuration file

User-Agent:       Mozilla/5.0 (Windows; U; Windows NT 6.0; en-GB; rv:1.9.1.3)
Gecko/20090824 Firefox/3.5.3 (.NET CLR 3.5.30729)

With 'MonoSetEnv <alias> MONO_IOMAP=all' specified in an Apache configuration
file for a precompiled site with a stylesheet file included in a theme a HTTP
500 error is returned if the case of the file in the theme and that on the
filesystem do not match e.g in the Theme the file is listed as 'UI.css' but on
the filesystem as 'ui.css'.  

Reproducible: Always

Steps to Reproduce:
1. Copy 'precompiled' files from attached test case to Mono environment.
2. Ensure the stylesheet file case on the filesystem is 'ui.css'.
3. Copy in and configure as appropriate the included apache configuration file. 
4. Browse to http://localhost/MonoTest

Actual Results:  
A HTTP 500 error is returned when 'UI.css' is requested (view with Firebug
running for example).

Expected Results:  
With MONO_IOMAP set to 'all' the case difference should not matter and the
stylesheet should be returned to the browser.

I have assumed that MONO_IOMAP applies to stylesheet files and not just
aspx,ascx etc. 

N.B This was tested with a precompiled site.
