Would returning the content of app_offline.htm with HTTP status 404 be a big
change? I think we should somehow signal to clients that the site is not
available.
This is not really when the client is a person that can interprete the content
of app_offline.htm.
But by returning HTTP status 200, you'll have - for example - web service
clients that try to parse the (app_offline) content as a valid SOAP response.
