[MonoTouch] Debugger won't connect to device

I'm debugging over USB with all the latest stable releases, and now the
debugger is never able to connect to the device. I've seen similar debugging
issues on the list, but no relevant answers, so I'm hoping additional info
will help.

The app's launch screen shows up, but then the app dies after the startup
time expires - and awhile after that, MD gives an error that "Could not open
any port for debugger. Another process may be using the port."

I can run the app normally on the device, just not while debugging. When I
start the app it vibrates at launch.

This is with a new phone, provisioning profile installed. I've tried
restarting, updating HostIP in MonoDevelopProperties.xml, etc.

Versions:
MonoDevelop 2.8.1
MonoTouch 5.0
Mono 2.10.6
iPhone 4s

My app's debug settings show:
Host: Automatic
Port: 10000
Output port: 10001
HeapShot Port: 10002
