[MonoTouch] Upgrade steps from iOS 4.x, Xcode 3.x and MT 4.x to iOS 5.0 and MT 5.0

Hi,
The only important order I can think of is to install MonoTouch after
Xcode, since the Xcode installer can remove /Developer (where MonoTouch is
installed) in some circumstances.
