[MonoDevelop] Building/Running MD From Source on Windows

I'm guessing you're running a 64-bit system, but there must be some x86
specific assemblies that get loaded - hence the BadImageFormatException.
Ignore me if I'm stating the obvious after the fact - just useful to
understand what's going on if you get these kind of problems in future.

I eventually realized that although no projects were selected to build with
the DebugWin32/AnyCpu configuration, the correct selection of projects were
enabled for the DebugWin32/x86 build configuration.  So that's the one that
needs to be used.  I also eventually realized that I had a *.vshost.exe
process running corresponding to a project that I was not currently working
with.  I killed that process, restarted visual studio, and launching into
debug mode started working.
