[MonoDevelop] Visual Studio Developer looking at MD

The should be a designer for GTK# files, which its one of the 'tabs' when
you open it. If it isn't there then the plugin might not have loaded. Check
the addon manager to see if it errored.

The controls should be in the toolbox when you open a GTK# file.  Custom
widgets need extra setup - the IDE needs telling which DLL to look in.
