[MonoDevelop] [PATCH] Rename refactoring patch part 1

Hi.

The attached patch implements the first part of rename refactoring.
Does this look ok to continue with or should it be done another way?

I tried to add the rename operation to the CommandInfoSet as the other
operations but I could not make the shortcut key work. That's why I
did it this way.

What needs to be done is more or less the GUI stuff.
