[Mono-list] Open sourcing an application

I have a Mono ASP.NET (though it should be compliant with MS .NET) 
picture gallery application that I have developed for my family and 
friends to be able to access and share pictures and galleries of 
pictures with other friends and families.  I'd like to open source this 
application but I'm wondering what the best way to do that is.

Is there a place that has a step by step guide to open sourcing an 
application?  I tried opensource.org but didn't see anything there like 
a how to guide.

Specifically I'm looking for:

- what types of headers do source files need
- what files need to be distributed with the source (I.E. any license 
files?)
- How to get "Open Source" certified.

Any help would be greatly appreciated.  Are there other things I need to 
consider?
