Paolo isolated the mcs bug.  The issue is that the scope of variables in
switch statements in the MS C# compiler live until the end of the switch
statement, while they are local to a switch section in Mono.

I need to fix that.  In the meantime, I was able to compile and run your
scheme interpreter by declaring d and z in the couple of places where
they were used.
