Attached is a revised version of the patch.  It changes the page ID tags
to match actual cref conventions, e.g. C:TypeName for constructors
instead of TypeName..ctor, etc.

It also adds support for more Mono.* namespaces on the
www.go-mono.com/docs site, so more types get linked there.
