The HH format *is* xhtml, so surely that makes it very related to xhtml?

Given that the format is pretty general, easy to parse, and presumably
well documented, it doesn't seem wise to create another format for the
same purpose when a good one already exists...

It's possible we'll need two providers - one that parses the HH TOC and
reads the docs from that, and another that builds a TOC simply from
spidering documents. At the moment though, I think it's best to
concentrate on the HH-oriented version that fits well for the mono
handbook...
