[Mono-docs-list] MonoDoc type lookups / helpsource improvement

Hello,

A few questions:

	* Is this the fix for delegates?

	* Is this the fix for the ref/out you talked about before?

	* We do not need to change the existing .xml files, only
	  re-assemble?
