generator/updater.cs?  I don't see anything in there that will modify 
existing XML nodes in the docs.  (It would need to modify the existing 
parameter nodes inside the existing member nodes and, say, add an 
attribute to distinguish ref and out.  Same problem for delegates.)

A custom program could be made easily enough, tho.
