I can confirm the bug as well as the workaround -- moving "Include
conf.d/*.conf" down about 5 lines. This is a RHEL issue.

This should be mentioned in the mod_mono setup docs.

While updating the docs, a simple apache-style "listing of all directives" for
the module would be greatly appreciated.

As would providing guidelines for 2.8 configuration as the 2.6 docs still talk
about threading and garbage collection workarounds and it's not clear which of
those recommendations still apply.
