Testcase

Maybe this Tescase helps you:

GTest.exe is the bug-673592_gtest.cs executable (the Debugger)
Test.exe  is the Debuggee.cs executable (the Debuggee)
AFDMProxy.dll is a simple stupid Assembly loaded into another AppDomain by the
Debuggee
Output.txt is the (silightly compressed) output.

You may notice, that the VMDeath event occurs prior to the AppDomain-Events
I haven't managed to reproduce the runtime crash message.
Maybe this crash only happens with Assemblys loaded via Byte-Array.
