MonoVS r8135 has the fix, it's now hitting the breakpoint.

You only need the changes in MonoVS, no Linux / runtime etc. changes needed.

I'm still seeing the problem with the 100+ threads sporadically, but I think we
can postpone this for the moment since these threads have a lifetime of less
than a second, so it should only be a performance problem.

I'll look at the other bugs after lunch.
