I did some testing over the weekend and managed to finally get the crash on
happen on one of our machines.

The key bit of information from this crash that none of our users provided is:

ERROR:threads.c:370:small_id_alloc: assertion failed: (id <
HAZARD_TABLE_MAX_SIZE)

I have had the crash 3 times on this machine - with same assertion each time.

I was testing a hashing implementation that lived in the C# side.

The crash happens after 3-5 hours of the application running.

I was checking the following every 15-30 minutes (and unless something peaked
right at the end) I didn't see any issues relating to memory usage (seem static
at around 300-400MB, threads were 25-35 and file descriptors were around 50).
