Patch to remove unloaded types from table

User-Agent:       Mozilla/5.0 (Macintosh; U; Intel Mac OS X 10.6; en-US;
rv:1.9.2.3) Gecko/20100401 Firefox/3.6.3

Type load events aren't emitted for types that are unloaded (with an appdomain
unload) and then reloaded.

This causes symptoms like breakpoints not being readded in reloaded types.

Reproducible: Always
