Backported the fix to 2.6 (commit 6d5b866863eb321c5faafc3ae6c5e94bad67fd54)

Additionally, I added code to HttpContext.RewritePath which deals with paths
containing consecutive slashes as well as code to the application pipeline
which throws exception if a handler for the request cannot be determined
(commit 4dcbe7950b18e271fdbc6f416b35b5d207b3a1f4 for master and
2c19129491dabff17197019c0c4a25bab458aa09 for the 2.6 branch)
