Andrew misunderstood Martin's comment in comment 4.

The problem is that the MonoVS server execs whatever Mono is in the path when
it launches the debugger.

Basically to fix this we have to either:
a) Source the Mono Addon Environment script in the MonoVS server script (as
suggested in comment 3).
b) Change the MonoVS server binary to locate the proper mono in managed code.

Andrew will be fixing this.
