Just a small annoyance, when I click 'Run' or 'Debug', the 'Build Output' tab
is always selected for building which is great, but as soon as the App actually
starts running it would be nice if the 'Application Output' tab could be
selected at that time.

Otherwise I often find myself clicking on the Simulator window, waiting for the
app to start, and then having to click back on MonoDevelop and then click again
to show the Application Output tab.

While we're at it, it would be nice if the iPhone Simulator could be 'brought
to the foreground' when the app is being started in the simulator too.
