Unfortunately I didn't include a nice simplified test case with my original bug
report and I don't have time to go back and make one right now. I do have some
extension methods in the code I was documenting, as well as non-extension
static methods and nested classes; maybe there's another bug in the code that
decides to insert the ExtensionMethods element in the first place.

For what it's worth, I hit this running the nant "doc" target of changeset
f188e55e9f of https://mvcutils.googlecode.com/hg/.
