ThreadExceptionDialog's icon is missing

REPRODUCE:
1. start uia2atk/test/samples/threadexceptiondialog.py

RESULTS:
you will not find the icon of the dialog

EXPECTED RESULTS:
there should be a icon on the left of the dialog
