Perhaps.  In that case, could you please try it with this line?
   e.PageSettings.PaperSize = new PaperSize("A4", 210, 297)
Your printer should complain about A4 paper.  If it does, that would be
sufficient to prove that Mono is working, rather than your system overriding
the settings with Letter.

Regardless of the result, this problem can be duplicated on the OpenSuse VM
image provided by the Mono project.  Merely install CUPS-PDF and print.  The
resulting PDF will be A4.  So it merely shifts the bug to "What could possibly
be overriding it, and why is it on the default OpenSuse VM?"  We first
experienced the problem on Debian Lenny (which has Mono 1.9 - Arggh Debian!) on
an HP Laserjet.
