I cannot reproduce the InvalidCastException on either trunk or the 2.0 branch.
Please provide a full, self-contained test case which triggers the error and
reopen the bug.

The Application_Start never called issue has been fixed in svn trunk, revision
116747 and in the 2.0 branch, revision 116748
