Updated patch and test, more correct but causes 20 test failures

The updated patch has a new test (passes on MS.NET) that gets a few more
details right (what events are raised and in what order, etc), and the
modifications to ToolStripItemCollection are designed to replicate that
behavior.

Unfortunately, this patch causes 20 test failures.  I tried messing with a
bunch of the internal magic between the ToolStrip* classes, but couldn't get
things just right.  Will have to revisit this.
