I never tried attaching event handler in Application_Start before, it was just
the fastest way to write the test case. I'll resubmit a proper test case
attaching on Init

1. Didn't know that but although a bug in mono, it's the same behavior 1.2.x
had.
2. Attaching on Init or from IHttpModule, the event handler gets executed,
RewriteUrl called, but it does not have any effect. This worked with 1.2.x and
works fine with MS
3. Same as 2. It get fired, and RewriteUrl called but does not have any effect
either.

I have verified this behavior in three different machines with 1.9.0 preview 4,
1.9.0 and mono-trunk. I'm not sure why you see paths properly rewritten :?
