New repro version including local faulty converter

As per the comments, this issue is un-reproducable as reported now that
ImageFormatConverter is fixed.  This new version of the repro app defines a
faulty TypeConverter locally and thus can reproduce/verify-fixed this 'multiple
errors' bug.  I see it fixed in trunk BTW.
