Your step (2) is wrong.  You wrote:

    monodocer -assembly:Library.dll -importshashdoc:Library.xml -path:doc

it should be:

    monodocer -assembly:Library.dll -importslashdoc:Library.xml -path:doc

(note that "-importslashdoc" was mis-spelled.)

If that isn't the problem, can you provide a copy of the source files you're
using that generates this problem?
