+XmlTextWriter does not implicitly output an xmlns prefix for an attribute 
+if the namespace is "http://www.w3.org/2000/xmlns/".

+I've added a test case named "XmlNs_Valid2" (marked NotWorking) to 
+XmlTextWriterTests.cs that allows you to reproduce this issue.
